What is ISBAR and what does it stand for?
It is a standardised framework for communication.
Introduction, situation, background, assessment, recommendation.
Outline the elements of ISBAR
Introduction: Who are you, your role, why are you communicating
Situation: What is happening at the moment?
Background: What are the issues that led up to this situation
Assessment: What do you believe the problem is?Recommendation: What should be done to correct the situation
Outline the 4 main stages thats involved in building a therapeutic relationship between HCP and patients
- Pre interaction phase (reparation (Clinical, Personal, Objectives)
- Introductory / orientation phase (Establish Rapport, Demonstrate Acceptance, Therapeutic Contract).
- Working phase (Patient Agency, Constructive Feedback, Implementation, Evaluation, Preparation for next phase)
- Termination phase (When mutually agreed goals are
reached, establish mechanisms for future needs)
How can we address the barriers to patient communication?
LACK OF PATIENT UNDERSTANDING AND ENGAGEMENT: Simplify language and use patient aids (e.g. show the product to the patient). Teach back method - (e.g. just to confirm your understanding, could you please explain to me what I said) OR “Just to make sure I have gone through everything clearly would you mind teaching me how to use the inhaler”

Describe the elements that promote effective goal setting
- SPECIFIC: Goals should have a clear and defined outcome
- MEASURABLE: Goals should be quantifiable, allowing progress to be tracked
- ATTAINABLE: Goals should be challenging but realistic, within reach of the Individual
- REALISTIC: Goals should be feasible, taking into account resources and limitations
- TIMELY: Goals should have a specific deadline, creating a sense of urgency
