purpose of confrontation field
relatively gross technique to screen for presence of visual field defect (central/ peripheral )
however this test is insensitive to subtle field defect
indication of confrontation field
gross screening test - part of routine comprehensive eye examination
useful will automated perimeter is not possible ; bed ridden px/ child
advantages and disadvantages of confrontation field

simple , quick , no special equipment and instrument needed
- too much interexaminer variability
- only able to detect large defect
- is not useful for monitoring ! too late
types of confrontation field and it’s purpose
facial amsler - detect central scotoma and scotoma in central field
finger counting - detects presence of absolute scotoma of each 4 quadrant
simultaneous finger counting - detect extinction phenomenon which may be present in parietal lobe lesion
simultaneous hand comparison - check for relative hemianopia , check for relative quadranopia altitudinal hemianopia , and nasal step
peripheral finger counting - compares examiner field to patient field
set up of confrontation field test
near light, remove spectacles (except facial amsler or high prescription px ) do test monocularly, sitting eye level and 60 cm away from patient
examiner to show fingers as target at 30 cm away
