Corneal epithelial defect associated with stromal infiltration commonly infectious process
Etiology of corneal ulcer ?
Bacterial - Common in CL : Pseudo , S Aureus , S pneumo
Virus - HSV , VZV
Fungus - Fusarium , Aspergillus , Candida
Protozoa - Acanthamoeba
Etiology that is not infection process ?
NK
EK
Autoimmune
Chemical injury
What is Clinical presentation of corneal ulcer ?
Eye pain
redness
Tearing
Photophobia
FB sensation
VA drop
Sign of Corneal ulcer ?
Signs
Corneal epithelial defect with underlying stromal infiltrate
Hypopyon (often in bacterial/fungal)
Edema, Descemet’s folds
Ciliary injection
Satellite lesions (fungal)
Dendritic lesions (viral)
Diagnosis ?
Corneal Scraping
Treatment strategy ?
- Bacterial infection
- Empirical > F.Cefazolin
- Moxifloxacin
-Fungal Ulcer - Natamycin or voriconazole
-Viral - antiviral Acyclovir , Ganciclovir
-Adjunctive - Tears , Cycloplegic
Surgical Indications ?
- Perforate + prone to perforate ( ex thinning )
