Explain your mgmt of MILD/MODERATE paediatric asthma.

A

Salbutamol pMDI and spacer

  • > 6yrs: 4-12 doses,
  • 2-5yrs: 2-6 doses,

4 breaths for each dose

Repeat at 20min intervals PRN.

Explain your mgmt of SEVERE paediatric asthma

A

Salbutamol and Ipratropium Bromide nedulised.

  • Small child (2-4yrs):
    2.5 Salbutamol,
  • Medium child (5-11):
    5mg Salbutamol,
  • Ipratropium Bromide 0.25mg.

Repeat (salbutamol) as required at 20min.

If no resolution/worsening symptoms manage as per CRITICAL.

Explain your mgmt of CRITICAL paediatric asthma

A

Increased dose of Salbutamol and Ipratropium Bromide nebulised.
Same dose across all paed age ranges.

  • 10mg Salbutamol,
  • 0.25mg Atrovent,
  • Repeat 5mg Salbutamol at 5min intervals.

AND,

  • Adrenaline (0.01mg/kg),
  • Repeat at 5min intervals PRN,
  • Max of 3 doses.
