cuspal coverage restorations Flashcards
(18 cards)
What is a cuspal coverage restoration?
A restoration that covers the cusps, may include the occlusal surface but not the mesial, distal, or buccal surfaces. Full crowns are also considered cuspal coverage restorations as they cover the entire tooth.
Why might a cuspal coverage restoration be appropriate? 5
To preserve the integrity of a weakened tooth
reduce cuspal deflection
increase protection of remaining tissue
correct occlusal discrepancies
during restorative reorganization of the occlusal scheme.
Examples include large access cavities, large occlusal forces, endodontically treated teeth, parafunction, and dentist’s knowledge.
when might a cuspal coverage restoration be appropriate?
Large access cavity with unsupported cusps
Large occlusal forces on tooth during function (if unopposed tooth may not need a crown)
Endodontically treated
Parafunction - clenching and grinding pt need cusps protected
Dentist knowledge and experience
remember
Study found that endodontically treated teeth not crowned after obturation were lost at a 6 x greater rate.
What are common designs of cuspal coverage restorations?
- Onlays
- Inlays with cuspal coverage
- 3/4 crowns
- 7/8 crowns
tooth preparation for a MOD inlay with cuspal coverage in gold
cavity walls:
cuspal coverage:
marginal finish
finish on functioning cusp
- Non-undercut cavity walls
- Cuspal coverage (1mm) with functional cusp bevel
- Bevel marginal finish
- Mini shoulder finish on functioning cusp
What are the advantages of indirect cuspal coverage restorations compared to full coverage restorations? 5
- Protects cusps
- Less sound tooth removed
- Less restorative material required
- Easier to inspect margins
- Potentially better aesthetics buccally
name an advantage of a 3/4 crown
Leaves an unrestored surface (buccal) so don’t see the metal
the design of preparation for a 3/4 crown is similar to a gold MOD. what is the difference?
proximal grooves that must be parallel to each other and slots should face palatally/lingually
What are the disadvantages of indirect cuspal coverage restorations compared to full coverage restorations? 3
- Complex preparation for clinician
- Complex restoration for technician
- More expensive than direct intracoronal restoration
What materials are used for the construction of cuspal coverage restorations?
- Indirect: Gold, all ceramic composite
- Direct: Composite
What types of cement are used for the cementation of cuspal coverage restorations?
- Luting cement (for retentive cavities)
- Bonding cement (for unretentive cavities)
Examples: Resin modified GI - Rely X luting, Poly F for luting; Resin - Panavia for bonding.
name 2 luting cements
Resin modified GI - Rely X luting
Poly F
name a bonding cement
Resin - Panavia - dental super glue
What is required for the maintenance of cuspal coverage restorations?
- Oral hygiene instruction (OHI)
- Diet advice
- Regular review
True or False: Full crowns are a type of cuspal coverage restoration.
True
Fill in the blank: Cuspal coverage restorations can help to correct _______ discrepancies.
[occlusal]
What is the study finding regarding endodontically treated teeth?
Endodontically treated teeth not crowned after obturation were lost at a 6x greater rate.
