Pregnancy referral criteria – 9
A
- Hyperemesis gravidarum (dehydrated, electrolyte imbalance)
- Severe unstoppable vomiting
- Inability to keep fluids/food down for 24hrs
- Very dark urination, or no urination
- Abdominal pain or fever
- Weakness or feeling faint
- Blood in vomit
- Persistent nausea & vomiting
- UTI

Morning sickness OTC advice - 7
A
- Avoid foods/smells which trigger symptoms
- Eat ginger or plain biscuits/cracker before getting up
- Eat small frequent meals, high in carbohydrates & low in fats
- Have plenty of rest since tiredness can make symptoms worse
- P6 pressure point
- Multivitamins or ginger may work
- Drinking little & often may help prevent vomiting

Pain management – 5
A
- Analgesics might be needed for headache, musculoskeletal pain or dental pain – Only give paracetamol
- Do not stand or sit for too long
- Rest when painful & ‘put feet up’
- Wear flat shoes
- Stretch tight muscles & soak in warm baths

Heartburn – 2
A
- 1st line: Avoid stooping, trigger foods, stop smoking
- 2nd line: Simple antacids/alginates, no PPIs

Heartburn referral – 4
A
- Symptoms unresponsive to treatments
- Patient is unable to eat due to symptoms
- Cause unrelated to pregnancy (peptic ulcer, IBS)
- Alarm symptoms present - GI bleeding, progressive unintentional weight loss, persistent vomiting, difficulty swallowing

Constipation – 3
A
- 1st LINE: increased dietary fibre, increase fluid intake, light exercise. If possible, stop medications which causes constipation.
- 2nd line: Bulk forming laxatives, refer if not working
- Avoid stimulants (e.g. senna) due to risk of uterine contractions

Haemorrhoids – 2
A
- Use of a cold compress are useful if the haemorrhoids are swollen and painful.
- Treatment – no one treatment has overwhelming evidence for effectiveness their product could be down to patient choice/past experience.

Vaginal thrush – 3
A
- Usually clears after delivery
- Avoid oral fluconazole
- If clotrimazole products are used – applicators should generally not be used

Coughs & colds – 5
A
- Avoid use of decongestants – use Menthol inhalation instead
- Care with combination preparations
- Paracetamol for symptomatic relief
- Some cough & throat lozenges can be recommended
- Speak to GP about Vicks vapo rub

Threadworms – 3
A
- Nothing OTC
- Change bed linen daily
- If symptoms appear refer

Problems during breastfeeding - 6
A
- Feeding related problems
- Sore/cracked nipples
- Blocked duct
- Too little/much milk
- Engorgement
- Thrush & breastfeeding
Nipple – miconazole cream after feeds
Mouth – miconazole oral gel >4 months only
