Clinical clues for ASD

- Right ventricular heave
- Wide, fixed splitting of the second heart sound
- Mid systolic pulmonary flow or ejection murmur
- Right axis deviation and incomplete RBBB


Important ASD complications

- Atrial arrhythmias (Afib and flutter)
- Stroke due to paradoxical embolization
- Migraine headache
- Pulmonary hypertension and Eisenmenger syndrome


Clinical features of CVID (common variable immune deficiency)

- Recurrent infections (rhinosinusitis, pneumonia, conjunctivitis)
- Chronic lung disease (bronchiectasis)
- Autoimmune disorders (vitiligo, RA, thyroid)
- GI disorders (IBS, malabsorption)
- Granulomatous dz
- Increase risk of malignancy (lymphoma, gastric CA)


Diagnosis of CVID (common variable immune deficiency)

Extremely persistent low IgG levels
Low IgA and/or IgM levels
No response to vaccinations
Exclusion of other immunodeficiency disorders


Treatment of CVID (common variable immune deficiency)

Immunoglobulin replacement therapy
Age-appropriate cancer screening
Monitoring for lymphoma


Adult CF

Mild respiratory infections, GI disease (recurrent pancreatitis, biliary cirrhosis, portal HTN), abnormal reproductive function


Myeloperoxidase deficiency

Asymptomatic or recurrent Candida infections


Primary ciliary dyskinesia

Nasal polyposis, otitis, recurrent respiratory infections, situs inversus (50% of pts), infertility


Onychomycosis diagnosis

KOH (potassium hydroxide) preparation from nail scrapings
