What is Frontotemporal lobar degeneration (FTLD)?
a pathological process that occurs in frontotemporal dementia.
There are 5 classifications based on the abnormally accumulating protein that’s present:
- FTLD-tau (includes Pick’s and FTDP-17)
- FTLD-TDP
- FTLD-UPS
- FTLD-FUS/FET
- FTLD-ni (no inclusions)
What are the two major phenotypes of FTLD?
Behavioral variant
Primary progressive aphasia (nonfluent, agrammatic, semantic, and logopenic)
Utilization behavior?
starting the ‘appropriate’ behavior associated with an object at an ‘inappropriate’ time. Often seen in FTD.
What is agrammatism?
Speech containing mainly content words. Person asked to describe kids playing in the woods: children.trees.run.
PPA nonfluent/agrammatic variant criteria? Where is the atrophy on MRI?
At least one:
- Agrammatism
- Halting speech w/ apraxia + 2 of the following: impaired comprehension of syntactically complex sentences, spared single word comprehension, spared object knowledge
Atrophy in left posterior frontoinsular area predominantly
PPA semantic variant clinical diagnosis? Area of atrophy?
- Impaired confrontation naming
- Impaired single word comprehension with at least 3 of these: impaired object knowledge, surface dyslexia or dysgraphia, SPARED repetition, SPARED speech production (grammar and motor)
Anterior temporal lobe
PPA logopenic? Area of atrophy?
- Impaired single word retrieval in spontaneous speech and naming
- Impaired REPETITION of sentences and phrases w/ at least 3 of these: speech (phonologic) errors in spon speech/naming, spared single word comprehension/object knowledge, spared motor speech, absence of frank agrammatism
Left posterior perisylvian or parietal atrophy
