What is Frontotemporal lobar degeneration (FTLD)?

A

a pathological process that occurs in frontotemporal dementia.

There are 5 classifications based on the abnormally accumulating protein that’s present:

  • FTLD-tau (includes Pick’s and FTDP-17)
  • FTLD-TDP
  • FTLD-UPS
  • FTLD-FUS/FET
  • FTLD-ni (no inclusions)

What are the two major phenotypes of FTLD?

A

Behavioral variant

Primary progressive aphasia (nonfluent, agrammatic, semantic, and logopenic)

A

starting the ‘appropriate’ behavior associated with an object at an ‘inappropriate’ time. Often seen in FTD.

A

At least one:

  • Agrammatism
  • Halting speech w/ apraxia + 2 of the following: impaired comprehension of syntactically complex sentences, spared single word comprehension, spared object knowledge

  • Impaired single word comprehension with at least 3 of these: impaired object knowledge, surface dyslexia or dysgraphia, SPARED repetition, SPARED speech production (grammar and motor)
