A

leaks from pulp to interstitual space, defence role
dififuclt to bond to

Q

tertiary dentine

A

in response to stimulus, laid down by odontoblast
mild - primary, severe - secondary odonto
plug function, seals tubules from access to pulp, protects vital tissue from exterior threat

Q

exchange of material between dentine and pulp

A

pulp -> dentine
- nutrients, formation of tertiary dentine
dentine -> pulp
- diffusion of toxins from bacteria, filling materials

Q

anatomy of pulp nerves

A

branches of alveolar, enter via apical foramen
fan out in sub-odontoblastic layer, some enter dentinal tbules
innervtes in tubles, less in coronal and root

Q

hydrodynamic mechanism

A

stimuli on surface of dentinal tubules
increase rate of dentinal flow, generating action potentials in intra-dental nerve, passed to brain as pain
ACTIVATION OF INTRA-DENTAL SENSORIAL NERVES, GENERATES PAIN
outwards/inwards
outwards - cooling/drying [GREATER STIMULI]
inwards - heating, pressure

Q

stimuli acting directly on intra dental nerves

A

intense heating/cooling, electrical current, pain producing chemicals, high fillings distorts dentine and alters tubular fluid flow

Q

intra-dental nerve properties

A

A beta/delta fibres [myelinated]
- activated by hydrodynamic stimuli
c fibres [unmyelinated]
- activated by stimuli directly, response, pain with pulp inflammation

dentine-pulp response to injury

A

immediate - pain
1m - inflammation vasodilation
10m - sensittisation
100m - enzyme
1day - nerve sprouting
1week - tertiary dentine
variable - completion of repair and recovery

Q

pulpitis

A

acute inflammation in pulp, but cannot swell due to pulp chamber
oedema increase in pulp pressure, variable effects on blood flow and nerve excitability
