Weekly education tool - would highlight gaps in knowledge
- help with patient safety, guidelines, staff cohesiveness, shared learning
- More guidelines
- Simulation and MDT working on LW - teaching and training for midwives
- example of difficult airway - hard to find guidance and report
- example of lost to follow up allergy testing
What is PSIRF?
Patient safety incident response framework
- idea is to learn from mistakes and improve
- better forms of data collection eg RADAR
- previous focus only on serious incidents leading to repetition
- oversight previously seen as punitive - more collaborative and no blame
- provider and oversight can work together to improve systems
- response plans are unique to each trust
- all involved in the incident are involved and invited to be part of solution
What is GIRFT?
Getting it right first time
National incentive
1 - data gathering and analysis - get picture of national practice
2 - clinical engagement - examine a trust and compare to national average
3 - guidance and support then provided for mainly high volume low complexity work
eg which equipment to use ie best practice
Issues for surgery
- improving ERAS
Integrating periop care across all surgical specialties
- focus on regional
- expansion of day case
- electronic anaesthetic charts
- collaboration and MDT training
What are issues with staffing DDH and paeds cover?
23k paeds attendances/yr
10-12 day need immediate resus
11 transfers to PGH in 1 yr 3 needing anaesthetics
Options
- more funding for cover in DDH
- centralise to PGH
- separate anaes DDH on call
How can productivity be managed in theatres?
Need anaes/surgeon/patient
AIm to use all 25 theatres
Productivety starts at booking/pre assessment
Ensure mix of patients listed to fill list
On the day productivity - staffing timing effeciency

Post op - day case bed availability PACU space and staffing
