What is thinking upstream vs. downstream?
Upstream- society as focus of change. Nurses think about social/political/economic influences on health. Advocacy is important.
Downstream- individuals as focus of change. Narrow view/put blame of disease on person.
What are determinants of health?
Broad range of personal, social, economic, and environmental factors that determine individual/population health.
What are SDOH?
Specific group of social/economic factors within determinants of health. Conditions in which people are born in/grow/live/work. Determines physical/social/personal resources a person has to achieve their goal.
Examples- discrimination, income, education, employment
What are health inequities?
Difference in health that are in fair, unjust, avoidable. SDOH influence these/are the factors behind them.
What are the 14 SDOH?
Income, education, unemployment, employment, food insecurity, housing, social exclusion, health services, indigenous status, social safety network, early childhood development, gender, race, disability.
INDG and SDOH?
Negative SDOH will intersect with each other to the detriment of health as evidenced by health inequity. Their income is lower, so if life expectancy, and they experience less chronic illness.
How does SDOH impact insituational racism?
Difference in access to goods, services, and opportunities of society because of race. It affects policy as well.
What is impact of SDOH on geography?
Rural and remote communities have decreased economic opportunity, access to healthcare, food, education. There is also environmental impacts of climate change and pollution.
What do nurses do r/t SDOH?
Develop broader perspective on health/illness, identify groups at risk, understand social conditions that affect health. Assess clients SDOH and consider them in treatment.
What is self determination?
Having freedom/authority within material/political realm. It will increase health outcomes.
Gender and SDOH?
Women have longer life expectancy but poor health/increased chronic diseases. Negative SDOH adversely impact women due to children and there’s discrimination in employment.
