Compounding Requirements

A

(1) Date of preparation
(2) Lot numbers
(3) Expiration date of the finished product. Date must not exceed 180 days or the shortest expiration date of any component in the finished product
(4) Signature or initials of the pharmacist performing the compounding
(5) Formula for the compounded product
(6) The name(s) of the manufacturer(s) of the raw materials
(7) The quantity in units of finished products or grams of raw materials
(8) The package size and the number of units prepared

Naloxone Furnishing

A

Pharmacist must have completed the training. Must screen for the following:
Currently uses or has a history of opioid use
Person is a friend or family member of someone who uses opioids
Determine whether or not they have an allergy to naloxone
Once dispensed, must counsel on it.
Give resources/information on addiction.
Provide training on how to recognize overdose and what to do. (Provide overdose prevention and recognition.)
Choose the correct route of administration
Correct labeling
Provide patient with a fact sheet
If given consent by patient, Pharmacist must notify PCP
Must be documented in med record
follow privacy policy of pharmacy

Travel Immunization Requirements

A

Complete a course at a minimum of 10 hours
Need to be Immunization certified.
yellow fever training course.
BLS certified.
CE every 2 years (need 30 hours every 2 years)
