Describe dimensions
- > they all interrelate to one another and create a holistic assessment of patients
- > define patients state of well being (overall view of general health)
What happens each time a nurse starts a shift, admits a new patient, take over care for a different patient, etc.?
-> assessment from head to toe, constantly assessing patients (eg. social, physical, etc.)

- introduction phase: introduce self / purpose of conversation / ask patient how they would like to be addressed
- working phase: data gathering phase / involves open and closed questions as well as responses to what the patient said
- closing & termination phase: ask client if there is anything else they would like to mention? / give a summary of the interview
