What is SPA
Single point of access
Telephone number for patients/HCPs to call which triages call to appropriate service
What is a package of care and the maximum allowance
Combination of services put together on hospital discharge to meet a persons care needs
Maximum:
2 carers QDS for 6 weeks, key lock, mobility aids
Difference between nursing home and residential home
Nursing - NHS funded, for medical needs, need continuing healthcare document to apply for funding
Residential - self funded
Who is involved in discharge planning
Doctor - MFFD, TTOs 14 day supply
Nurse
PT/OT - fit for ADLs, any equipment or modifications needed for house
Social worker - provides POC
What are section 2 and 5 for social workers
2: lets social worker know patient will require a POC
5: 48 hour notice for social services to have POC ready
When do patients pay for their POC
If savings/property over a threshold
AND
Require continuing care after maximum 6 weeks (6 weeks free for all)
What is the continuing healthcare checklist used for
Decides if patient needs a continuing POC
Decides if POC should be funded by NHS or social services
