Why is noting co-morbidities on discharge summaries so important?
It impacts the amount of impact the Trust generates from treating a particular condition.
When writing discharge forms, think about some useful things to include for the GP…
Discharge tests results (e.g. bloods, scans)
What happens to the purple DNAR forms after discharge?
Patients are given a copy to take away with them, which is then kept in the fridge so that paramedics can find it.
How long does a discharge summary take to reach the GP?
3 weeks
What should you do if you need something (e.g. a test) before 3 weeks time?
Ring the GP and tell them, then make note of that on the summary.
Also put some onus on the patient to book appointment etc.
What is the medication section on the discharge summary for? And what things should you think about?
The medications that the patients is LEAVING hospital with (not coming in with). And what you want them to be on for the next 30 days isn.
If you have changed a patient’s medications, you need to note that and make clear why you have done it.
Also if patient NEEDS to be on a drug for a long time for a particular time, make sure you note that very clearly in block capitals.
