A

Group 1

1st fit – inform DVLA – 6 months

Epilepsy/abnormal EEG – increased risk – 12 months

Diagnosed epilepsy – seizure-free for 12 months

Group 2

1st fit (& no antiepileptics used): 5 years

epilepsy – seizure-free off medications for 10 years

TLOC:
- vasovagal / syncope w trigger/reversible cause?
- unexplained syncope (incl w no trigger)?

(group 1 vs group 2 for both)

A

vasvagal / triggered
- group 1: while standing: can drive & DON’T need to inform DVLA. while sitting: must not drive for 4 weeks (inform DVLA if not resolved)
- group 2: no driving for 3 months && inform DVLA

Unexplained syncope (sitting or standing):
- group 1: 6 months (if CV & treated - may drive after 4 weeks)
- group 2: 12 months (if CV & treated - may drive after 3 months)

nb if recurrent unexplained syncope then group 1 12months, group 2 10 years

time also greater if cough syncope

Other neuro conditions which need to inform DVLA (aside from strokes, TLOC & seizures)

some can still drive with

  • narcolepsy
  • MS / MND / PD (any chronic reduction in strength / coordination)
  • ‘disabling dizziness’
  • amyloid spells
  • PRES / RCVS
  • visual inattention (barred)
  • carotid artery stenosis (group 2 only)
  • encephalitis / meningitis
  • pituatory tumour
  • brain tumours / cysts
  • metastatic brain tumours
  • brain biopsy
  • traumatic brain injury (post-traumatic amnesia >24hrs, seizures, haematomas or contusions)
  • subdural haematoma
  • SAH
  • intracranial aneurysm
  • AV malformation
  • intracerebral abscess
  • chiari malformation
  • hydrocephalus
  • DBS etc insertion

What 3 things do pts with visual impairment have to prove before can have licence back (group 1)?

What about group 2?

A

number pate @ 20m, peripheral fields 120 degrees, diplopia needs controlled

Group 2: may need to go to DVLA

Driving rules for ACS? (Group 1 & 2)

Incl if need to inform DVLA

Rules if CABG?

A

Group 1:
- no driving for 1 week if successful revascularisation & LVEF >40%
- if no revasc or EF worse: ban for 4 weeks
- DON’T need to inform

Group 2:
- MUST inform DVLA
- may be reinstated after 6/52 if revasc & LVEF >40%

Nb same w elective PCI

If CABG: 4 weeks (group 1) or 3 months (group 2)

Driving rules for arrythmias

Group 1 &2

A

Eg sinoatrial node, significant AV block, AF/flutter, narrow or broad tachy

Must not drive if arrythmia has caused or likely to cause incapacity (unable to control or stop a vehicle)
Group 1: no inform
Group 2: inform

Can drive again if underlying cause been identified and controlled for 4 weeks (group 1) or 3 months (group 2)

Other cardiac conditions with driving restrictions

A
  • post-catheter ablation (2 days-3/12)
  • pacemaker implant (1-6 weeks)
  • complete heart block (until pacemaker)
  • ICD (all must notify, group 1: 1/12-2years, group 2: permanent ban)
  • aortic aneurysm (depends on size)
  • aortic dissection (all notify)
  • malignant HTN
  • HOCM (group 2 only)
  • heart failure: ejection fraction <40% (permanent ban for group 2)
  • LV assist device
  • heart transplant
  • pulm HTN (both must notify)
  • SYMPTOMATIC/severe heart valve disease (group 2 and AS in group 1) & post-surgery
  • symptomatic congenital heart disease
  • long QT syndrome
  • brugada syndrome

Insulin-dependant diabetes driving rules group 1? 4 points

Group 2??

A

INSULIN

ALL MOST NOTIFY DVLA

Group 1:
- adequate awareness of hypos
- no more than 1 episode of severe (need assistance from others) hypo while awake in last 12 months (and none in last 3 months)
- appropriate glucose monitoring (before and every 2hrs)
- meets visual standards for driving

Group 2:
- same as group 1 but no severe hypo in 12months and license reviewed annually

Nb can use libre for group 1, but not group 2

Non-insulin dependent diabetes rules

A

For sulfonylureas: same rules as insulin

If other tablets: group 1 don’t need to notify, group 2 do need to notify but norm no problem

(If diet controlled: no notify or restrictions)
