Early Childhood Caries Flashcards
(19 cards)
ECC definition
the presence of 1 or more decayed, missing, or filled tooth surfaces in any primary tooth in a child 71 months of age or younger
S-ECC definition
in a child < 3 years: any sign of smooth surface caries
from age 3-5: 1 or more cavitated, missing, filled smooth surfaces in the Mx primary anterior teeth
ECC etiology
bacteria
tooth
substrate
ECC bacteria
parent/caregiver transmission to child
ECC teeth
structure
spacing
ECC substrate
bottle to bed
WHO recommendations on breadtfeeding
initiate within the first hour
exclusively breastfed for first 6 mo
breastfed on demand
no bottles, teats, pacifiers
start other foods at 6 mo.
continue to breastfeed until age 2 years and beyond
sudden infant death syndrome ways to lower risk
back to sleep
prevent exposure to tobacco
sleep in approved Crib/cradle/Bassinet
share room with parent
breastfeeding
ECC breastfeeding etiology
breast milk alone is not thought to be cariogenic however in combination with other fermentable carbohydrates can definitely be associated with the development of ECC
early signs of ECC
white spot lesions
indicate need for referral to dentist
treatable with: improved oral hygiene changes to feeding/diet and fluoride
fluoride varnish
minimal risk of toxicity
applied by dentist
every 1-3 months
spread of ECC
follows eruption sequence
incisors then molars
dental abscess/fistula/gum boil
no antibiotics
treatment is extraction
facial cellulitis of dental origin
look for dental cause
assess for dehydration/fever
manage with antibiotics
needs surgical intervention
how do we tx children with ECC
clinic: LA, sedation, protective stabilization, operating room
what is the most common reason preschool children require general anesthesia
dental procedures
at risk populations for ECC
low SES
medical co-morbidities
children requiring nutritional supplements
dietary risk factors
past history of decay/fillings/extractions
canadian fluoride recommendations
no fluoride supplements for children or pregnant women
amount of toothpaste: less than age 3 (grain of rice), over 3 years (green pea)
water fluoridation
optimal water fluoridation 0.7 mg/L
balancing risk of dental fluorosis vs. benefits to teeth
many communities are taking fluoride out of their water supplies
