Regurgitant flow is maximal at the beginning of systole and decreases with increasing left atrial pressure
-> decrescendo configuration is common.
Mitral regurgitation
May be seen in primary tricuspid regurgitation with normal right ventricular systolic pressure (ex:
infective endocarditis in drug addicts)
Early systolic murmur with a decrescendo configuration
Tricuspid regurgitation
In large VSD with pulmonary hypertension, the murmur may be early systolic in timing, since the
increasing right ventricular pressure during late systole decreases the left-to-right shunt; findings of
pulmonary hypertension are always present
