- CHF
- cirrhosis
- renal failure
- hypoalbuminemia
Causes of Euvolemic true hypoNa+
- SIADH
- hypothyroidism
- low solute intake, etc.
Causes of hypovolemic true hypoNa+
renal & extrarenal losses
Causes of normal osmolality w/ hypoNa+ = pseudonatremia
Incr PRO or lipids in blood
Causes of high osmolality w/ hyponatremia = pseudohyponatremia
- Hyperglycemia
- Recent mannitol or sorbitol use
What do we check when assessing volume status?
-Vital signs
-Orthostatics
-JVP
-Skin turgor
-Mucous membranes
-Peripheral edema
Main causes of SIADH
- pulm malignancy
- symptomatic HIV
- Drugs
- Idiopathic
Main antidepressant drugs that cause SIADH
- SSRIs
RFs of Osmotic Demyelination Syndrome (ODS)
(central pontine myelinosis)
- Serum Na+ < 105 mEq/L
- Chronic hypoNa+ corrected too quickly
- EtOH use
- Malnutrition
- Liver dz
