why motivational interviewing is useful tooth for oral health ?
improving oral health is required behaviour changes. it is help to prevent periodontal disease. However, change behaviour is very challenging. motivational tools is to help focusing on exploring what the pt think about changing their behaviour and to understand what they see or think about their situation. it is clinician guidelines change. patient centred and determine the feeling of ambivalence about change
what is the reasons patient do not change behaviour?
past life experiences: attitudes , beliefs and habits develop over time
current circumstances; lack of time and money, health literacy and knowledge
confidence and skills
how the communication style that reduces motivation?
directive style where the dental hygienist acts as expert and patient is the recipient. which may causing the pt become defensive, ignore the advice or delay return treatment
how communication that increase motivation
by using motivational interviewing (MI); focuses on exploring what the pt thinks about changing their behaviour a and patient entered.
what are the four key elements of MI and describes It
partnership: avoid expert role. patient and hygienist work collaboratively
acceptance: the hygienist respect the pt autonomy, strengths and perspective
compassion: the hygienist keeps the pt best interest in mind
evocation: the concept that the best ideas about change come from the pt
what are the five core skills of MI
open-ended questions: avoid simple yes or no questions. encourage pt to elaborate to learn the pt perspective
reflective listening: the health care provider listens, then paraphrase what they heard the pt says. check if you are getting the message right. validate the pt.
affirm: acknowledge the pt for taking small steps towards the behaviour change. and affirms the pt strengths and efforts.
summarise: reinforce what has been discussed, and provide opportunity for the clinician to demonstrate empathy , can move conversation in new direction
provide information and advice: decide when providing more information is effective and avoid pitfall of pressing for change with the pt. inquire what the pt already knows, provides relevant information and advice and check back with the pt to see what he or she has take from the information . avoid lecture
what are the four processes of MI? and describe it
engaging: encourage a collaborative dialog with the pt. elicit and listen to the pt perspective and used open ended questions, used reflective listening and affirm the pt for any strengths
focusing: clear goals or direction in mind, clarify with the pt what the direction of the conversation will go, clinician may need to provide more information.
evoking: want to evoke their internal motivational about change, want to elicit their confidence about making a change. clinician need to facilitate and strengthen this desire for change
planning: the clinician can begin planning in small steps for change, guide the pt in how the change will occur, and try to evoke intention and commitment from the pt
