A

Indications:

  1. Directional pedals do not move, or if they do, do not effect a change in yaw
  2. Yaw movement related to changes in collective
  3. Aircraft may experience sudden excursions in pitch, roll, and/or yaw

Procedures:

  1. *Controls - ADJUST (Use power and airspeed to align the nose).

  3. Rotor brake handle not stowed
  4. Decrease in Nr

Procedures:

  1. *Rotor brake handle - STOWED

If warning light remains illuminated and seconday indications exist

  1. *HYD 1 PBA - OFF (illuminated)
  2. *COOLER PBA - HYD 2 (illuminated)
  3. *Nr - MONITOR

A

Indications:

  1. HYD 1 or 2 TEMP HI caution message and aural tone
  2. Hydraulic pressure normal

Procedures:

  1. *HYD 1/2 HT EXCH and HYD CONT circuit breakers - IN

If Circuit breakers are in and hydraulic temperature exceeds 115ºC:

  1. *Applicable hydraulic system failure - EXECUTE

A

Indications:

  1. Grinding or howling noise from pump
  2. Fluctuating or low hydraulic system No.2 pressure.
  3. HYD 2 PRESS LO caution message and aural tone.
  4. HYD2 FLUID LVL LO caution message
  5. ROLL SCAS and AFCS disengaged
  6. MRGB TEMP HI and/or CBOX TEMP HI caution message(s) and aural tone if the COOLER PBA is in the HYD 2 position.

Procedures:

  1. *Establish airspeed less than 100 KIAS and angle of bank less than 35 degrees.
  2. *COOLER PBA - HYD 1 (blank)

  2. Fluctuation or low hydraulic system No. 1 pressure.
  3. HYD 1 PRESS LO caution message and aural tone
  4. HYD 1 FLUID LVL LO cuation message
  5. PITCH and YAW SCAS and AFCS disengaged
  6. MRGB TEMP HI and/or CBOX TEMP HI caution message(s) and aural tone if the COOLER PBA is not in the HYD 2 position.

Procedures:

  1. *Establish airspeed less tha 100 KIAS and angle of bank less than 35 degrees
  2. *COOLER PBA - HYD 2 (illuminated)

A

Indications:

  1. Smoke
  2. Fumes
  3. Fire
  4. ENG 1 or 2 FIRE warning on flight page, ENG1 or 2 fire light on glareshield, and aural tone.
  5. ENGINE NUMBER 1 or 2 FIRE voice message
  6. Smoke trail observed during yaw maneuver.

Procedures:

  1. *Controls - ADJUST (to establish single-engine parameters).
  2. *Stores - JETTISON (as approriate)
  3. *Throttle (affected engine) - CLOSE
  4. *ENG fire PBA (affected engine) - ARM
  5. *READY/DISCH PBA - DISCHARGE. Illumination of the READY indicates fire bottle is ready for discharge.

Ensure indication changes to DISCH to show fire bottle discharged.

6.*FUEL PBA (affected engine) - CHECK CLOSED.

  6. Engine instruments for both engines decrease
  7. RPM warning light and aural tone
  8. ROTOR RPM voice message.

Procedures:

  1. *Autorotation - ACCOMPLISH

A

  1. ENG fire PBA (affected engine) illuminated and voice alert
  2. Smoke
  3. Fire

Procedures:

  1. *Throttles- CLOSE
  2. *START PBA’s - OFF (blank)
  3. *FUEL 1 and FUEL 2 PBA’s - CLOSE
  4. *ENG fire PBA (affected engine) - ARM
  5. *READY/DISCHARGE PBA- DISCHARGE. Illumination of the READY indicates fire bottle is ready for discharge. Ensure indication changes to DISCH to show fire bottle dishcarged.

A

Indications:

  1. Affected Engine bands/pops
  2. No throttle response
  3. Decreasing or erratic Ng
  4. Increasing or erratic MGT

Procedures:

  1. *Controls - ADJUST (to maintain Nr, reduce power, and establish single-engine parameters)
  2. *Stores- JETTISON (as appropriate)

Engine Np Underspeed

A

Indications:

  1. Left Yaw
  2. Nr/Np droops if power demand exceeds single-engine limit
  3. Np mismached with Nr at 100 percent or slightly drooped.
  4. MGT decrease
  5. Ng decrease
  6. Torque split

Procedures:

  1. *Controls - ADJUST (to control Nr)
  2. *Stores - JETTISON (as appropriate)

Engine Ng/Np Overspeed

A

Indications:

  1. Right yaw
  2. Increase in Ng, Np, and Nr
  3. increase in MGT
  4. Torque Split
  5. ENG 1/2 Np or Ng OVERSPEED caution message and aural tone
  6. Possible rotor RPM warning light, ROTOR RPM HI warning message and aural tone.
  7. The good engine reduces power and Np of the overspeeding engine may increase well over 100 percent.

Procedures:

  1. *Controls - ADJUST (to control Nr)

Engine Hot Start

A

Indications:

  1. MGT reaches 867ºC during engine start

Procedures:

  1. *Throttle (affected engine) - CLOSE
  2. *FUEL PBA (affected engine) - CLOSE
  3. *START PBA (affected engine) - ENGAGE. motor until MGT decreases below 145ºC or 2 minute automatic timeout is reached

Single-Engine Failure During Takeoff

A

Indications:

  1. Left Yaw
  2. Nr decreases
  3. Engine indications (affected engine) decrease.
  4. ENG 1/2 OUT (affected engine) and RPM warning lights illuminated with voice messages
  5. ENG 1/2 OUT (affected engine) or RPM LOW warning legend is displayed on the FLT page warning banner

Procedures:

If Fly-away airspeed does not exist:

  1. *Controls - ADJUST (cushion aircraft for landing)

If fly-away airspeed does not exist:

  1. *Controls - ADJUST (to climb and establish/maintain autorotation parameters)
  2. *Single-engine failure in flight procedures - EXECUTE

  5. *ENG fire PBA(s) - ARM
  6. *READY/DISCH PBA - DISCHARGE Illumination of READY indicates fire bottle is ready for discharge

Ensure indication changes to DISCH to show fire bottle discharged.

  1. *Autorotative landing - ACCOMPLISH

A

Indications:

  1. Somke or fumes
  2. Equipment failure
  3. Fire
  4. High AMPS indication

PROCEDURES:

  1. *Affected equipment - OFF and CBs pulled.

If fire persists:

  1. *INV 1/2 PBAs - OFF (illuminated)

If fire persists:

  1. *GEN 1/2 PBA’s - OFF (illuminated)

If fire persists:

  1. *Land immediately

Complete Electrical Failure

A

Indications:

  1. Loss of the non-essential electrical bus and associated mission computer and cockpit displays.
  2. Battery voltage drains.
  3. Loss of essential bus components to include the remaining mission computer and cockpit displays.

PROCEDURES:

  1. *Check BATT PBA - ON (blank)

if battery power is not restored:

  1. *Land immediately

A

Indications:

  1. APU fire PBA illuminated and voice alert.
  2. Smoke
  3. Fire

Procedures:

  1. *APU - OFF (blank)
  2. *APU fire PBA - ARM
  3. *READY/DISCH PBA - DISCHARGE. Illumination of READY indicates fire bottle is ready for discharge

Ensure indication changes to DISCH to show fire bottle discharged

  1. *Helicopter - SHUTDOWN

Indications:

  1. PRI OIL COOLER FAIL caution message and aural tone
  2. Rise in CBOX and MRGB oil temperature
