  • Error is a crew action or inaction that leads to a deviation from normal operations.
  • Flight crew errors can be the result of
    – induced by an expected or unexpected threat
    – momentary slips, lapses, or mistakes

Slips and lapses

A

are failures in the execution of the intended action.

Slips are actions that do not go as planned ,

while are lapses memory failures. For example, operating the flap lever instead of the (intended) gear level is a slip. Forgetting a checklist item is a lapse.
