Errors
- Error is a crew action or inaction that leads to a deviation from normal operations.
- Flight crew errors can be the result of
– induced by an expected or unexpected threat
– momentary slips, lapses, or mistakes
Two categories of Errors
a.) Slips and lapses
b.) Mistakes
Slips and lapses
are failures in the execution of the intended action.
Slips are actions that do not go as planned ,
while are lapses memory failures. For example, operating the flap lever instead of the (intended) gear level is a slip. Forgetting a checklist item is a lapse.
Mistakes
are failures in the person’s plan of action.
Even if execution of the plan were correct, it would not have been possible to achieve the intended outcome.
