determing movement reduction via pain vs tear
lidocaine injection test removes pain, can’t fix a tear.
Tx of shoulder tear
NSAIDs, subacromial injection, surgery
DO NOT use sling
Dx rotator cuff tear
lidocaine injection test, MRI
frozen shoulder
stiffened glenohumeral joint, loss of range of motion
Dx of frozen shoulder
ROM tests:
Touchdown
Apley Scratch
shoulder impingement syndrome definition
compression of tendons and bursa between humeral head and acromion process
DDx shoulder impingement vs rot. cuff tendonitis
strength preserved in shoulder impingment
Tx frozen shoulder
treat underlying cause, self limiting
Dx of shoulder impingement
hallmark: painful flexion-internal rotation maneuvers
Tx shoulder impingement
physical therapy, ice, NSAIDs, activity modification
NO sling
AC joint injury definition
sprain, partial or complete tear
Dx AC joint injury
enlargement of joint, tenderness, pain with adduction/downward traction
grade 1 AC joint injury
slight stretch, no gross instability, unremarkable Xray
grade 2 AC joint injury
AC ligament tear, partial CC ligament tear. Partial dislocation
grade 3 AC joint injury
complete dislocation, clavicle elevation, usually requires surgery
Tx AC joint injury
only one that can go into a sling. reduce AC pressure. Ice, rest, NSAIDs
