Final Exam Notes Flashcards
(13 cards)
TMJ
Osteoarthritis main finding
crepitus
Afferent pain fibers from C1-C4 have projections through the _________ as it coordinates sensation from the ophthalmic, maxillary and mandibular portions
spinal nucleus of the trigeminal nerve
Internal Derangement:
- Clicks present ‼️
- Functional dislocation of the disc anteriorly with reduction
- Functional dislocation without reduction (clicks not present, limitation of motion to rotation, no translation, motion limited to 25mm opening
Psychological TMJ disorder main findings:
- Up-regulation of central nervous system, trigeminal nucleus
- Those with TMJ tend to be over-reactive to their environment
- Anxiety, depression, anger, fear all tend to increase with chronicity and may contribute to increased pain
Lumbar Spine
Internal Derangement Disc Dysfunction:
Stage I
- disc slightly anterior
- inconsistent click
- mild or no pain
Internal Derangement Disc Dysfunction:
Stage II
- disc anterior reciprocal click
- painful
Internal Derangement Disc Dysfunction:
Stage III
reciprocal click, now later during opening and earlier during closing, most painful stage
Internal Derangement Disc Dysfunction:
Stage IV
click rare, disc no longer relocates, stays anterior, pain rare
TMJ
Is there a C-shaped? or an S-shaped? deviation to movement?
- C-shaped: hypomobility on side of direction
- S-shaped: motor control problem
Atlanto-occipital joint primary motions:

- Extension 10 deg
- Flexion 5 deg
- Small SB: 5 deg
- no axial rotation
Atlanto-axial joint primary motions
- Axial rotation 35-40 deg
- Extension 10 deg
- Flexion 5 deg
- no SB

Lumbar Spine
the best test to rule in lumbar radiculopathy
Crossed Straight Leg Raise (Sp 95%)
the best test to rule out lumbar radiculopathy
Straight Leg Raise (Sn: 97%)
