Humeroulnar head - Medial epicondyle of humerus (via common flexor origin), ulna collateral ligament, medial part coronoid process of ulna

Radial head - proximal ⅔ anterior border of radius

Insertion: Splits into 4 tendons as it crosses wrist joint; each tendon splits as it passes over palmar surface of proximal phalanx of all fingers to insert into sides of the bases of the middle phalanges of each finger.
