bipolar depression meds

A

Quetiapine (monotherapy)
Antipsychotic with good evidence for bipolar depression.
Sedating → helps with sleep.
Start low, titrate gradually to 300 mg daily (or more as needed).

Olanzapine + Fluoxetine (Symbyax)
Effective combo but weight gain and metabolic issues common.
Use if monotherapy not effective.

thorough risk assess - e.g. lashing out at children, make it personal

OCD go in straight away with SSRIs

no leading questions

offer to clean chair, ask if needs doctors note
