How does one acquire cryptococcos?

A

inhalation of yeast-like cells from soil contaminated w/ pigeon droppings
- not dimorphic, always an encapsulated yeast

How can one Dx cryptococcosis?

A
  • CSF of meningitis pts - treat w/ India ink, look for halos

- CSF testing for antigens

A
  • amphotericin B and flucytosine combo therapy fro CNS cases

A
  • Exserohilum rostratum- always a mold

diagnosed by CSF culture and PCR

What is the treatment of steroid-associated meningitis?

A
  • IV voriconazole for 3 months or more
  • severe cases should get Amphotericin B
  • injection recipients who are Asx should be observed
