Upper GI bleed - what action to take if GBS 0, 1, 2-7 (stable), 7+ or unstable?
0 - discharge from ED
1 - dicuss with Gastro SpR
2-7 - admit gastro - endoscopy within 24h
7+ - Urgernt gastro review and emergency OGD
What Hb value should you consider transfusion in upper GI bleed?
<80 - transfuse
80-100 discuss with gastro Reg
NB If suspect variceal bleeding - only transfuse if <80 as can encourage bleeding
What prokinetic agent can be given before endoscopy in upper GI bleed?
Erythromycin
What two medications are often given in maangement of variceal bleeding?
- Terlepressin
2. Tazocin
How do bulk forming laxatives work and give an example?
Work by adding water to the stool and thus volume
Isphagula husk, Fibergel
How do osmotic laxatives work and give an example?
Add water to BOWEL to soften stool?
Movicol (also stimulant), Docusate
How do stimulant laxatives work and give an example?
Induce colon muscles to induce a bowel movement
Senna
What medication can be used in the treatment of PSC?
Ursodeoxycholic acid
What liver marker is raised into 1000s with viral hepatitits?

- US and doppler
- Hep A, B, C , EBV, CMV
- IgG, anti smooth muscle Ab, ANA –> AI hepatitis
- IgM, anti mitochondrial Ab –> primary sclerosing cholangitis
- IgA - raised in alcohol abuse
- Anti trysinogen (Alpha 1 antitrypsinogen)
- Ferritin (Haemachromatosis)
- Caerusplatin (Wilsons)
