Propofol

A

2, 6 di iso prophenol; non barbituate induction (soy, gylcol, eggs)
MOA: discourages dissacoation of GABA from GABAa receptors. Chloride conductance results in hyper polarization of the post synaptic membrane that produces hypnosis; inhibition of glutamate at NMDA receptors
Onset: 30 seconds
Volume cmpt: 3
redistribution: 2-8 minutes
e1.2time: 1.5 hours
DOA short
metabolized: in liver by CYP 450 (gluronide and sulfate)
excreted: renal
S/E: pain on induction, decreased bp, svr, myocardial depression, propofol infusion syn (long term treatment with sedatives and anesthetics in children or crucially ill that are also receiving glucocorticoids and catecholamine. rhabdo/hyperkal/renal failure; tx: support and d/c proposal)
CI: hypersenstivity to soy or egg, hypovolemic and hypotensive pt, decrease dose in the elderly
indcution 1-2.5mg/kg iv, NV 10 MG, ga 100-300 mcg/kg min, sedation 25-100 mcg/kg/min
