Colchicine MOA
- binds to tubulin, decreases microtubular polymerization
- decrease formation of LTB4
- inhibits leukocyte and granulocyte migration
Colchicine Adverse effects
Diarrhea, GI pain, peripheral neuropathy , hematuria, alopecia
Allopurionol MOA
Inhibit xanthine oxidase and decrease purine metabolism and therefore iron acid
Allopurinol adverse effect
Peripheral neuropathy, stone formations, rash, vasculitis
Febuxostat MOA
Non purine selective inhibitor
Inhibit 6MP
Febuxostat adverse effect
GI upset, arthralgia, HA, rash
AVOID: if patient on Theophylline, Azathioprine or 6MP
Probenecid MOA
Inhibit proximal tubule reabsorption or urate
Inhibit secretion of acidic drugs
Probenecid Adverse effect
High excretion of urate –> urate crystals in kidney
Aspirin antagonizes the uricosuric action and interferes with clearance of methotrexate
Sulfinpyrazone MOA
Inhibit proximal tubule re absorption of urate(like Probenecid)
Inhibit secretion of acidic drugs
Inhibit platelet aggregation
Sulfinpyrazone Adverse effect
GI distress, rash, NEPHROTIC SYNDROME
