Heart and Neck Vessels: Subjective Data
Chest pain Dyspnea Orthoopnea Cough Fatigue Cyanosis or pallor edema Nocturnal Past cardiac history Family cardiac history Patient-centered care (cardiac risk factors)
Subjective data: Chest pain
Any chest pain or tightness? Brought on by activity? Any associated symptoms? Aggravating factors? Treatments?
Subjective data: dyspnea
Any SOB? Onset? Duration? Aggravation? Timing/ Treatment? Orthopnea?
Subjective data: cough
Any cough? Duration? Frequency? Type? Any mucus? What does it sound like? Aggravating factors?
Subjective data: fatigue
Any tiredness?
Onset?
Timing?
Subjective data: cyanosis or pallor
Ever notice skin turning blue or grey?
Subjective data: edema
Any swelling of feet or legs? Time of day? How much swelling? Where? Does it go away? Any associated symptoms? Aggravating factors? Anything make it better?
Subjective data: nocturia
Do you wake up to urinate?
Subjective data: past cardiac history
Any history of HTN, elevated cholesterol, triglyceride, heart murmur, congenital heart disease, rheumatic fever
Last ecg?
Family have any heart disease?
Summary checklist: heart and. Neck vessels examination
Neck: carotid pulse (observe and palpate), observe jugular venous pulse, estimate jugular venous pressure
Precordium
Inspection and palpation: describe location of apical impulse, note any life or heave
Auscultation:
- Identify anatomical areas where you listen
- Note rate and rhythm of heartbeat
- Identify S1 and S2 and note any variation
- listen in systole and diastole for any extra heart sounds
Listen in systole and diastole for any murmurs
Repeat sequence with bell
Listen at the apex with person in left lateral position
Listen at the base with person in sitting position
