Takotsubo cardiomyopathy
= broken heart syndrome
Stress induced cardiomyopathy, looks like Octopus on CMR
Pathophys HOCM
predominantly diastolic dysfunction
LV hypertrophy -> decreased compliance - decreased output
“myofibrillar hypertrophy with chaotic + disorganized fashion myocytes (“disarray”) + fibrosis on biopsy”
HOCM presentation
jerky pulse, large A waves, double apex beat
systolic murmurs
- ejection systolic - LV outflow obstruction
- pansystolic - due to systolic anterior motion of mitral valve !!
What can increase \ decrease the ejection systolic murmur heard in HOCM
increases - valsava manoevre
Decreases - on squatting
other conditions assoc with HOCM
Friedreichs ataxia
Wolff Parkinson white
HOCM echo findings

mitral regurg (MR)
