CAP in pts <60 yrs?
1) Azithromycin or clarithromycin, doxycycline
2) Fluoroquinolones (2nd line)
CAP in pts >60 yrs, with comorbidities?
1) Levofloxacin or moxifloxacin
2) 2nd or 3rd gen cephalosporin
All are first line agents
Hospital acquired pneumonia?
Any of the following:
- Ceftazidime or cefepime
- Imipenem
- Piperacillin + tazobactam
- do NOT use macrolides (only in CAP)
Lung abscess?
- Gram positive = ampicillin, amoxicillin/clavulanate, ampicillin/sulbactam, vanc for staph
- Anaerobes = clinda or metronidazole
- Gram negatives = quinolones or ceftazidime
PPD positive without active dz?
9 mo INH
Active TB?
4 drug regimen for 2 mo, then 4 mo of INH and rifampin
Meningitis in infants <1 mo?
Ampicillin + gentamicin OR cefotaxime
Meningitis in 3 mo - 50 yrs?
Vancomycin + ceftriaxone
Meningitis in >50 yrs?
Vancomycin + ampicillin + ceftriaxone
Meningitis in HIV?
Vancomycin + ampicillin + ceftazidime
Viral encephalitis?
Support
HSV = acyclovir
CMV = ganciclovir or foscarnet
Hep A and E?
Supportive
HBV?
INF-a, tenofivir
HCV?
IFN-a and ribavirin
Botulism?
Support resp status, antitoxin (toxoid) ASAP
Uncomplicated acute cystitis?
1) Oral TMP/SMX or nitrofurantoin first line
2) Fosfomycin or cipro
Complicated acute cystitis?
Ampicillin, amoxicillin, or oral cephalosporins (avoid quinolones for fetal arthropathy)
Uncomplicated pyelonephritis?
1) Gram negatives = TMP/SMX or fluoroquinolones
2) Gram positives = amoxicillin
A single dose of ceftriaxone or gentamicin is given intitially before starting oral tx
Complicated pyelonephritis?
IV ampicillin + gentamicin or cipro
Acute prostatitis?
TMP/SMX or fluoroquinolone and doxycycline
Chronic prostatitis?
Fluoroquinolone
Chlamydia?
1) Azithromycin (single oral dose)
2) Doxycycline (oral for 7 days)
Gonorrhea?
1) Ceftriaxone (single IM dose)
2) Give azithromycin (one dose) or doxycycline (7 days) for coexistent chlamydia
PCP?
TMP/SMX + prednisone
