What are the two infectious lung diseases?
- Pneumonia (bacterial or viral)
- Tuberculosis
What is Pneumonia?
An acute inflammation of the lungs with alveolar consolidation
What is the pathophysiology of pneumonia?
Infectious agent (bacteria or virus) which reaches the lung and causes a inflammatory reaction, may also be the result of an auto immune disease
What is the aetiology of pneumonia?
- Aspiration
- Contact (trauma or chest tube)
- Inhalation (droplet)
- Hemtogenous (circulation)
Pneumonia clinical presentation?
- Inspection: cyanosis, fever, increased RR + shallow breathing, productive cough, dyspnea
- Palpation: increased tactile fremitus
- Percussion: dull
- Auscultation: inspiratory and expiratory crackles, decreased BS
- ABGs: decreased O2, decreased CO2 (increased RR)
- CXR: opacities in surrounding alveoli
What is Tuberculosis?
Infectious, systemic, inflammatory disease primarily affecting lungs and other organs
What is the aetiology of Tuberculosis?
Mycobacterium tuberculosis (airbourne precaution)
Clinical presentation of Tuberculosis?
- Respiratory: Cough (> 2 weeks), dry (early), productive (mucus/BLOOD***)
- Systemic: fever, fatigue, night sweats, weight loss, may have swollen lymph nodes
Dx Tuberculosis?
TB infection: TB skin test, TB blood test
TB disease: CXR, Sputum sample, medical Hx (where have they travelled recently), phys exam
Tuberculosis interventions?
- Medication (NOT PT SCOPE)
- Secretion clearance techniques
- Deep breathing
- Coughing
- Prevent exposure to others (negative pressure room, N95 mask when entering room)
