Assessment of the skin provides data about:
Oxygenation Circulation Nutrition Tissue integrity Hydration Hygiene
Normal assessment of skin:
Evenly pigmented; no petechaie, no lesions, purpura, or excoriations
Warm, good turgor
Normal assessment of Nails:
Pink, oval, adhere to nail with 160 degree angle
Normal assessment of Hair:
Shiny, full; amount and distribution appropriate for age, no flaking of scalp or forehead, no pinna
Light skin cyanosis
Blue
Dark skin cyanosis
grey
Ecchymosis light skin
Dark purple, red, green, yellow
Ecchymosis dark skin
purple to brownish black
Erythmea light skin
reddish tone, possibly increased skin temp
Erythmea dark skin
Deeper brown or purple skin with increased skin temp
Jaundice light skin
yellow color
Jaundice dark skin
yellow green color most noticeable in sclera of eye
Pallor in light skin
pale skin appears white or ashen
Pallor in dark skin
lack of underlying red tone, yellow brown skin
Petechiae light skin
small red-purple lesions
Petechia dark skin
difficult to see, may be in mouth or conjunctiva of eye
Rash light skin
visualized and felt with light palpation
Rash dark skin
Not visualized but felt with light palpation
Scar light skin
Generally heals, shows narrow scar line
Scar dark skin
Keloid development resulting in thickened raised scar
