intro to implant prosthodontics 2 Flashcards
(24 cards)
sequence for an implant
plan and consent
placement of implant
uncover and connect abutments
restore
monitor and maintain
how is the implant itself placed?
raise flap
place implant
place cover screw
suture
how are the abutments connected to the implant?
uncover implant
remove cover screw
place abutment
take wash impression
place temporary
cast impression
an implant crown can be retained by what 2 ways?
cement retained
screw retained
name an advantage of a cement retained crown
no hole in abutment
name 2 disadvantaged of cement retained crowns
can be difficult to remove as very retentive
excess cement around margins can lead other inflammation and bone loss
name an advantage of a screw retained crown
easily unscrewed if broken
name a disadvantage of a screw retained crown
avoid the screw access hole being labially as causes negative aesthetics - careful implant placement
name 4 ways a denture can be secured to implants
locator abutments
ball abutments
gold bar
CAD-CAM titanium bar
locator abutments ? slowly but are easily ?
deteriorate
replaced
how do ball abutments work
like a ball and socket joint
silver cap and gold insert on the denture that holds the ball part in the implant
ball abutments fail over time but ?? can be replaced easily
gold inserts
how does a gold bar secure a denture?
implants are joined together with a gold bar
gold clips on the denture clip into the bar between implants
advantage of gold bar
stops anterior rotation more than locator and ball abutments
2 disadvantages of gold bar
expensive
bar can be hard to clean
2 advantages of CAD-CAM titanium bar
cheaper than gold bar
less likely to fracture than gold as stronger and don’t have a solder line
disadvantage of CAD-CAM titanium bar
bar can be hard to clean
name 4 common post implant treatment complications
peri-implant mucositis
peri-implantitis
loose/fractured components
late implant failure
what is peri-implant mucositis?
inflammation of the soft tissues around an implant with no bone loss
what does peri-implant mucositis look like clinically?
tissue is red, swollen and may bleed on gentle probing
management of peri-implant mucositis
OHI and reassess at future visit
what is peri-implantitis
inflammation of the tissues around an implant and bone loss
management of peri-implantitis
refer to clinician who placed them
if can’t
- OHI and assess in 1-2months
- if no improvement get advice from secondary care
- if stable arrange for radiographic follow up in 6-12 months
what are the role of GDPs in implant pts ? 3
OHI - ensure plaque control, remove calculus and excess cement
examine peri-implant tissues for signs of inflammation, suppuration and bleeding on probing
take baseline periapical using long cone paralleling technique after 1year
