A
  • advise that only limited information is available. Where they seek further information they should make a request to the Accident Records Office
  • confirm that a collision has been reported and the circumstances of the collision outlined in the report are consistent with the request
  • ensure a TIS report has been completed
  • ensure the person completes the Form 1146 and Stat Dec
  • record the requested information on the Form 1146 and provide a copy to the requesting person
  • inform the person that the information may only be used for an authorised purpose
  • complete the remaining fields on the Form 1146 and forward all documentation to the ARO for recording and filing
