ITB friction syndrome
ITB contracture and friction syndrome can present
unilaterally & bilaterally
what is the IT band
reinforced tract of fascia connecting proximal to the TFL and glute max and distally to lateral tibia
- provides lateral support at knee, escpec when knee is extended
causes of ITB contracture
- activities that place knee and hip in flexion
- postural imbalances (APT)
- prolonged WC use or bed rest
causes of ITB friction syndrome
- prolonged repetitive activities where hip and knee and flexed (running)
- poor TFL stretching
- anterior pelvic tilt
symptoms
- pain at lateral thigh into lateral aspect of knee
- shortening IT band
- thickened, adhesion and fibrositic nodules
- inflammation due to IT band rubbing over lat.femoral condyle
- HT and TPs in TFL, hip flexors, rec fem
- valgus / hyperextension of knee possibly
- trochanteric bursas can develop
pain present contracture vs friction syndrome
contracture = P achy on lateral aspect of thigh, gradual onset
friction syndrome = P directly over lateral femoral condyle, worse on activity
with a unilateral IT band contracture viewed anteriorly, what tilt is seen
lateral pelvic tilt that is low on affected or contracted side
which movement of the knee is always painful w ITB friction syndrome
extension of knee
what does a positive Ober’s test indicate
shortened TFL or ITB
what does a positive Noble’s test indicate
presence of ITB friction syndrome
