What is patella Mal-alignment?
A
An increase in retro-patella pressure and osteochondral changes, caused when the patella translates or deviates from the moral groove

Common causes
A
- Muscle imbalance where vastus medialis is weaker than Lateralis causing patella mal-tracking
- Increase or change in training load
- Reduced quad or glute strength or altered muscle function
- Altered movement control, increased knee valves
- Decreased flexibility in quads, ITB, calf etc.
- Altered joint ROM
- Increased hip adduction during running

Signs and symptoms
A
- Anterior pain of the knee joint during exertion or load
- Pain increased downhill/stairs
- Tenderness on compression of the patella
- Positioned more laterally in comparison to the other knee

Treatment
A
Ischemic pressure to address TrP’s in the VL and TFL
- STR and MET to lengthen VL
- MET to reduce hypertrophy

Lateral pull test
