Ottawa Knee Rules
Positive: must have one of the following--- 1. over 55 2. isolated patella tenderness 3. tenderness to head of fibula 4. unable to flex knee to 90 5. Unable to weight bear 4 steps Indicates: send for xray
Milking test
Positive: unusually puffy or springy
Indicates: intra articular swelling
Patella Quadrant
Positive: increased movement Indicates: Medial=medial patellofemoral ligament or superior lateral retinaculum Lateral= superior medial retinaculum cephalic= infrapatellar tendon caudal= quad tendon
Sag test
Positive: sag of tibia compared to femur
Indicates: PCL injury
Posterior Drawer
Positive: reproduction of pain or empty end feel
Indicates: PCL injury
Anterior Drawer
Positive: reproduction of pain or empty end feel
Indicates: ACL injury
Lachman’s
Positive: reproduction of pain or empty end feel
Indicates: ACL injury
Valgus Stress Test
Positive: recreation of pain, excessive movement, empty end feel
Indicates: MCL injury
Varus Stress test
Positive: recreation of pain, excessive movement, empty end feel
Indicates: LCL injury
Apley’s distraction
Positive: recreation of pain, restricted or excessive movement
Indicates: MCL (with ER) or LCL (with IR)
McMurray’s
Positive: pain, snap, locking
Indicates: medial and lateral menisicus
Steinman’s
Positive: medial pain elicited on lateral rotation and lateral pain solicited on medial rotation
Indicates medial and lateral meniscus
Thessaly
Positive: pain, locking or catching
Indicates: Medial and lateral meniscus
Apley’s compression
Positive: recreationg of pain, restricted or excessive motion Indicates: lateral meniscus (with ER) Medial meniscus (with IR)
