what are some important considerations when anaesthetising CHILDREN vs ADULT and OLDER ADULT jaws?

A
  • the jaw of the child is much smaller, less dense, the location of the mandibular foramen is lower
  • the deciduous molar roots are more flared than the permanent counterparts.
  • The older jaw may also be less dense, and have variations in topographic landmarks depending on the tooth loss and alveolar bone resorption

explain LA to a child.

A
  • “I’m going to put some sleepy juice around the teeth, and when it wakes up it will feel better, look prettier/ wont feel ouchie anymore”
  • “I think your teeth wants to take a nap whilst i clean it.”
  • “you may notice something different to when the tooth goes to sleep” it wont stay like that, you will notice it will go away in an hour or so.

taste of LA

A

bitter and strong minty taste of the juice, but we will wash it away and use the straw/ vacuum which feels noisy, but it feels tickly.

what is not recommended for children?

A

Articaine- not recommended for children under 4
mepivacaine not recommended in under 30 yrs old.
- they can use lignocaine

what are some post operative instruction?

A
  • lip trauma, bite, soft tissue lesions are a common post operative complication in children
  • hard to remember which side of mouth is numb, so we give stickers on the chest of the side (parents can quickly observe them and quickly see if they are chewing/ biting/ playing with/ pulling their lip on the side thats numb- and gently remind them to stop.

Describe the procedure for an inferior Alveolar Nerve Block.

A

Nerves targeted: the IAN nerve+ the lingual nerve
area anaesthetized- the mandibles teeth pulp and roots on the ipsilateral side
