Definition of patient care
Providing care that is respectful of, and responsive to, individual patient preferences, needs and values, and ensuring that patient values guide all clinical decisions
Pickers eight principles of patient centred care
Access to care, Continuity and transition, involvement of family and friends, emotional support, physical comfort, information and education, coordination and integration of care, Respect for patients’ preferences
Respect for patient values, preferences and expressed needs
Involve patients in decision making Everyone has their own unique values and preferences Treat patients with dignity Respecgt their cultural values Respect their autonomy (want for help)

Coordination of clinical care
coordination of ancillary and support services (eg. Prescription sent home )
Coordination of front line patient care ( eg what we do)
Information and education
When communicating with healthcare provdiers, patients can sometimes feel as though they are not being completley infomred about their condition or progress. Especially in the types of terms that they are able to understand
Physical comfort (three most important areas)
Pain management
Assistance with activities and daily living needs (ex. Brush teeth , hair, basic needs)
Hospital and surroundings and environment (ex. Flowers)
Emotional support and alleviation of fear and anxiety
Fear and anxiety associated with illness and procedures can be as debilitating as the physical effects
Involvement of family and friends
Providing accommodations for family and friends
Involving family and close friends in decision making
Supporting family members as caregivers
Recognizing the needs of family and friends
Continuity and transition
Patients may be concerned about their ability to care for themselves
Understandable, detailed information regarding medications, physical limitations, dietary needs, etc.
Coordinate and plan ongoing treatment and services after discharge
Provide information regarding access to clinical, social, physical and financial support on a continuing basis
Access to care
Access to hospital, clinic and physician locations
Availability of transportation
Appointment availability and ease of scheduling
Accessibility of specialists
Clear instructions on how and when to get referrals
