Long-term care (LTC)
Is given in long-term care facilities for people who need 24 hour skilled care
Skilled care
Is medically necessary care given by a skilled nurse or therapist it is available 24 hours a day
It is ordered by doctor and involves a treatment plan
Length of stay
The number of days, a person‘s stay in a healthcare facility. It may be short such as a few days or months or longer than six months
Terminal illness
This means that the illness will eventually cause death
Chronic conditions
This means that the condition last along period of time even a lifetime
Ex) heart disease, dementia
Diagnosis
Medical conditions determined by a doctor
Home healthcare
Healthcare provided in a person‘s home usually given to older or chronically ill people
Assisted-living
These are for people who need help with daily task is showering meals, and dressing as well as medications they do not need 24 hour care
Adult day services
Are used for people who need help and supervision during certain hours, but do not need full care
Acute care
24 hour skilled caregiver in hospitals and ambulatory surgical centers
Subacute care
Care given in hospitals or long-term care facilities
Outpatient care
Is given to people who have had treatments, procedures or surgeries and needs short term skilled care
rehabilitation
Is care given by specialist and professionals, physical, occupational and speech therapist to help restore or improve function after an illness or injury
Hospice care
Is given in facilities or homes for people who have six months or less to live
Activities of daily living( ADLs)
Personal care includes bathing, skin, nail and hair, care, mouth care, and assistance with walking eating and drinking dressing transferring an elimination
Person centered care
Emphasizes The individuality of the person who needs care and recognizes and develops the person’s capabilities
Trauma informed care
Is an approach to patient care that recognizes that people may have experienced trauma in their lives trauma can come from witnessing or experiencing abuse or neglect, violence, prison, or military combat
Medicare
Four people 65 or older also covers people with permanent kidney failure or certain disabilities
1) helps pay for care in a hospital or skilled nursing facility
2) helps pay for doctor services and other medical services and equipment
3) allows, private health insurances companies to provide Medicare benefits
4) helps pay for medication’s prescribed for treatment
Medicaid
People who have low income or disabilities
People must qualify for this program
Charting
Recording important information about the resident
Nursing assistant
Performs tasks assigned her such as measuring vital signs also providing or assisting with personal care such as bathing and elimination needs
Registered nurse or RN
Coordinate managers and provide skilled nursing care, giving special treatments and medication’s as prescribed by a doctor also assigned task and supervise his daily care of residence
Licensed practical nurse
Gives medication’s and treatments
Has completed one to two years of education, has passed a national licensing exam
Advanced practice, registered nurse
(APRN)
In advance practice, registered nurse who has completed training at the postgraduate level, can make diagnosis and prescribed treatments
