What is classified at a massive haemorrhage?
- loss of one blood volume in 24 hours
- loss of 50% circulating volume in 3 hours
- loss of 150ml/min
How to estimate circulating blood volume?
Women: 60-70ml per kg
Men: 65-76ml per kg
How soon is O- blood available?
Immediately
How long does group specific blood take to be available? (ABO + RhD)
15 mins
How long does it take for fully cross matched blood to be available?
35 minutes
How long does it take for fresh frozen plasma + cryoprecipitate to thaw?
Up to 20 mins
General response to major haemorrhage protocol
- control bleeding - apply pressure
- venous access
- avoid hypothermia > warm fluids
- immediate bloods
- nominate on person to liaise with blood bank
What immediate bloods are taken in major haemorrhage protocol?
- FBC
- cross match
- coagulation screen
- biochemistry incl Ca
- blood gases if appropriate
Complications during massive haemorrhage
- hypothermia
- hyperkalamia
- acidosis
- hypocalcaemia
Warfarin reversal in major haemorrhage -
- stop warfarin
- IV vitamin K
- Human Prothrombin Complex
- Fresh Frozen Plasma if HPC not available
How long does it take for IV vitamin K to work?
4-6 hours
How long does it take for oral vitamin K to work?
24 hours
