A

Threats are events that occur beyond the influence of the flight crew, increase operation complexity and must be managed to maintain margin of safety.

eg; Adverse metrological conditions, airports in mountains, aircraft malfunctions, errors from outside the Flight Deck.

A

Actions or inactions by the flight crew that lead to deviations from organisation or flight crew intentions or expectations.

eg; Inability to maintain stabilised approach, wrong automation mode, failing to give required call-outs, misinterpreting ATC clearance.
