Threats are events that occur beyond the influence of the flight crew, increase operation complexity and must be managed to maintain margin of safety. eg; Adverse metrological conditions, airports in mountains, aircraft malfunctions, errors from outside the Flight Deck.

Actions or inactions by the flight crew that lead to deviations from organisation or flight crew intentions or expectations. eg; Inability to maintain stabilised approach, wrong automation mode, failing to give required call-outs, misinterpreting ATC clearance.
