Supine to sidelying on intact side

A
  1. pulls to sidelying with intact arm
  2. Moves leg across actively and lower body follows but arm is left behind
  3. Lifts arm across with intact arm. Leg is moved actively and rest of body follows in a block
  4. Arm is lifted across actively and rest of the body follows in a block
  5. Moves arm and leg actively but overbalances (shoulder protracts and arm forward)
  6. Rolls to side in 3 seconds (must not use hands)

Supine to sitting over side of bed

A
  1. Sidelying, patient lifts head but cannot sit up
  2. Sidelying to sitting over side of bed (therapist assists but patient controls head)
  3. Sidelying to sitting over side of bed (therapist assists legs)
  4. Sidelying to sitting over side of bed (stand by help)
  5. Supine to sitting over side of bed (no stand by help)
  6. Supine to sitting over side of bed within 10 seconds (no stand by help)

Balanced sitting

A
  1. Sits only with support (therapist assists into sitting)
  2. Sits unsupported for 10 seconds (no holding on, knees and feet together, feet on floor)
  3. Sits unsupported, weight well forward and evenly distributed (head and thoracic spine extension)
  4. Sits unsupported, turns head and trunk to look behind (hands resting on thighs, don’t allow to touch plinth)
  5. Sits unsupported, reaches forward to touch the ground and returns to starting position (must reach at least 10cm in front of feet, may support affected arm if necessary)
  6. Sits unsupported on a stool, reaches sideways to touch the floor and returns to starting position (may support affected arm if necessary)

Sitting to standing

A
  1. Gets to standing with help from therapist (any method)
  2. Gets to standing with stand by help (weight unevenly distributed and uses hands)
  3. Gets to standing (no uneven weight distribution or help from hands)
  4. Gets to standing and stands for 5 seconds with full hip and knee extension (do not allow uneven weight distribution)
  5. Sitting to standing with no stand by help (must have full extension of hips and knees)
  6. Sit to stand with no stand by help 3 times in 10 seconds

Walking

A
  1. stands on affected leg, takes a step forward with other leg (weight bearing hip must be extended, therapist may give stand by help)
  2. Walks with stand by help from 1 person
  3. Walks 3m alone or using any aid but no stand by help
  4. Walks 5m with no aid in 15 seconds
  5. Walks 10m with no aid, turns around, picks up a sand bag, walks back in 25 seconds
  6. Walks up and down 4 steps with or without an aid but without holding onto a rail 3 times in 35 seconds

Upper arm function

A
  1. Lying, protract shoulder with arm in elevation (therapist places arm in position and supports elbow in extension)
  2. Lying, hold arm extended in elevation for 2 seconds (physio places it in position and arm is maintained with some ER, must remain within 20 degrees of full extension)
  3. Flexion and extension of elbow to take palm to forehead (therapist may assist supination)
  4. Sitting, hold extended arm in forward flexion (90 degrees) for 2 seconds (therapist places arm in position and patient must maintain position with some external rotation and elbow extension, do not allow excessive shoulder elevation)
  5. sitting, patient lifts arm to 90 degrees forward flexion and maintain for 10 seconds then lower (don’t allow pronation)
  6. standing, hand against wall, maintain while turning body to wall

Hand movements

A
  1. sitting, extension of wrist (do not allow elbow flexion)
  2. sitting, radial deviation (no elbow flexion or pronation)
  3. sitting, elbow to side, pronation and supination
  4. reach forward, pick up large ball of 14cm diameter with both hands and put it down (shoulder protracted, elbows extended, wrist neutral or extended)
  5. Pick up polystyrene cup from table and put it down on opposite side of the table
  6. continuous opposition of thumb and fingers more than 14 times in 10 seconds (do not allow thumb to slide or move backwards)

Advanced hand activities

A
  1. Picking up top of pen and putting it down again
  2. Picking up one jellybean from cup and placing in another cup (8 jellybeans in one cup, both cups at arms length, left hand takes jellybean from cup on right and places into left)
  3. Drawing horizontal lines to stop at a vertical line at least 10 times in 20 seconds (at least 5 lines must touch and stop at vertical line)
  4. Holding pencil, rapid dots (at least 2 dots per second for 5 seconds, patient must pick up pencil without assistance, holds as for writing, makes dots not strokes)
  5. Take a dessert spoon of liquid to mouth (do not allow head to lower to hand and do not allow spills)
  6. Hold a comb and comb at back of head
