What are FP10CDFs?
Since 2015 an approved form must legally be used to requisition Sch2 and 3 CD
When are FP10CDFs not needed?
Hospitals when do not need to use approved forms if request is from a ward or department
What are requisition requirements/legal requirements for what needs to be written on a Sch2 or 3 CD requisition?
- name of recipient
- signature of recipient
- address of recipient
- their profession of occupation
- total quantity of drug
- purpose of the requisition
What should requisitions be like?
- faxed/photocopied requisitions are not legal
- received before supply made
- emergency - supplied within next 24 hrs
- keep 2 years, send original to NHSBSA
- vets - keep original 5yrs
Who may use MSO?
- registered midwife
- to obtain diamorphine, morphine or pethidine
What must MSO state?
- name of midwife
- occupation
- name of person, administered the CD
- purpose why CD needed
- quantitty issued
- signature of appropriate medical officer, doctor or supervisor of local midwives
What are prescription requirements for Sch2 and 3 CDs?
- signature
- date
- prescribers adress
- dose, formulation, strength
- total quantity
- quantity prescribed
- name of patient, address
what do dates need to be like?
- date of signing
- valid for 28 days
- applies to Sch4 CDs
- applies to owing balances
what do doses need to be like?
clearly defined
what do formulations need to be like?
- stated even if only one exists
what does the prescriber’s address need to be like?
- included on prescription
- in the UK
what do strengths need to be like?
- only needed if more than one strength exists
- if more than one on prescription, all need to be stated
what does total quantity need to be like?
- written in words and figures
- in dosage units then express as dosage units
what are prescription requirements like for Sch 2 and 3?
- in instalments - valid instalment direction required
- mark prescription with DoS = “spent”
What are private prescription for Sch2 and 3 CDs like?
- standardised form used
- not required for hospitals if for itself
- prescriber id num included
- send to NHSBSA
- dont use for non CDs
