Clonal hematopoiesis (CH) definition (5th who)

A
  • population of cells derived fr a mutated multipotent stem/progenitor cell — got selective growth advantage in absence of unexplained cytopenia, hem cancers or other clonal disorders
    ⬆️ overall mortality, cv ds and myeloid malig
    VEXAS syndrome (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic UBA1 mutations) : manifes of interplay between inflam & CH/myeloid neoplasia that are being gradually uncovered
    precursor myeloid ds state

(CHIP) definition (5th who)

A

CH + somatic mutations of myeloid malig-assoc genes in blood or BM with vaf >=2% (>=4% for x linked gene mutation in males)I’m ppl w/p hem malig or unexplained cytopenia

Clonal cytopenia of undetermined significance
(CCUS) definition (5th who)

A

CHIP + >=1 persistent cytopenia that can’t explained by hem or non-hem comditioms & don’t meet dx criteria for defined myeloid neoplasms

Hb<13 g/dL (males), Hb<12 g/dL (females); ANC <1.8 x10^9/L ; plt < 150 x 10^9/L
