Syncope
Fainting, temporary loss of consciousness.
Anemia
Condition in which there are low red blood cells (can cause fatigue and shortness of breath)
Heart Attack
Medical emergency that occurs when blood supply to the heart is blocked.
Hypoglycemia

Condition in which the heart is unable to pump enough blood to meet bodies needs. (too weak)
Angina
Chest pain or discomfort caused by reduced blood flow to the heart. Results of atherosclerosis.

Medical emergency that occurs when blood supply to a part of the brain is disrupted.
Ischemia
Lack of blood flow to body part or organ.
Myopia
Nearsightedness, individual has difficulty seeing objects that are far away.
Arrythmia
Refers to irregular heartbeat.
Advance beneficiary notice (ABN)
Form provided to the patient when provider believes medicaid will not pay for services rendered
Allowed Amount
Maximum a third party payer will pay for a particular procedure and service
Copayment
Amount of money paid at time of medical service
Coinsurance
A policy provision frequently found in medical insurance whereby the policyholder and insurance company share the cost of covered losses in specified ratio ex 20:80
Deductible
Specific amount of money a patient must pay out of pocket before insurance begins paying
Explanation of Benefits
Statement from insurance detailing what paid, denied, or reduced in payments; contains info about amounts applied to deductibles, coinsurance, and allowed amounts.
Participating Provider (PAR)
Providers who agree to write off the difference between the amount charged by provider and approved fee established by insurer.
Managed Care
Umbrella term for plans that provide health care in return for preset scheduled payments and coordinated care through defined network of providers and hospitals.
CMS-1500 Form
Standard paper claim form used by non institutional providers & suppliers to bill Medicare, Medicaid, and other government/some private insurance entities.
