Q

What is Graded Assertiveness

CUSS

A

C Concern
= I’m concerned that allergies haven’t been recorded
U Uncertain
= I’m uncomfortable/unsure, but this dose seems high for a child
S Safety
= I dint think it’d safe to proceed until we make dure this is the correct dose
S Stop

Q

A

After a Sentinel event to determine WHY the error happened
= cause and effect
= cause of error (not error itself)
(Not who.)
= fish bone diagram

Q

What is FMEA
FAILURE MODE EFFECT ANALYSIS

A

Prospective
Identifying and preventing product problems before they occur

Doesn’t require previous bad experienced

Used more in engineering and manufacturing of devices

Health care = HFMEA
BUT decidability = smaller emphasis (as not always possible)
